Electronic Forms

This help page introduces and explains the new electronic forms that have replaced the Office of the Registrar's paper forms. Now, rather than stopping by the Registrar's office, you can just click on the desired tasks within myCU portal to initiate certain requests. 

To Locate the Forms:

1. Navigate to myCU portal. 
2. Search for the task using the Search bar, you will see an option for each of the forms listed below. 
3. Click on the task for the form you wish to access.

To View Links to the Forms: 

• Change a Major, Minor, and/or Concentration Change
• Request an Advisor Change
• Request a Course Substitution
